UI Health Welcomes LGBTQ+ Patient Navigator
A new LGBTQ+ Patient Navigator is now available to receive referrals of LGBTQ+ patients in need of complex care management. Referrals may also be made for LGBTQ+ training for staff.
LGBTQ+ Patient Navigator Complex Care Management is designed to help high-risk patients and caregivers manage multiple, ongoing medical conditions and co-occurring psychosocial risks that are likely to last more than 30 days.
This service is available to any UI Health LGBTQ+ patient and involves a Social Determinants of Health risk assessment, a patient-centered care plan and regular, proactive outreach.
